Claims Process

  1. Gather verifiable observations: sights, sounds, smells.
    • Who: Individuals involved and witnesses. 
    • What: Description of incident. 
    • When: Date and time. 
    • Where: Specific location with photos, room numbers, and addresses. 
    • Why: Alleged cause of incident. 
  2. Submit the Incident Form. 
  3. Email additional info as it becomes available to [email protected]. 
  4. Keep report details confidential and direct inquiries to [email protected].
